Jennifer Trevino, OTR/L, CAPS, ECHM is an Occupational Therapist and Aging In Place Specialist with 18 years of experience in a variety of settings including hospitals, rehab centers, outpatient, skilled nursing facilities and home health. Through working with clients in different settings, she has seen the meaning and importance of staying in one’s own home to benefit their overall health.
As an Occupational Therapist, I have worked with clients with many different medical conditions. My clinical training helps me to assess how an individual performs their daily activities. While every situation is unique, I can appreciate what is important to each client to help find ways to ensure that their home meets their needs. I am passionate about improving the safety and quality of life for my clients. Over the years, I have seen falls limit one’s return home potential. Falls are the leading cause of unintentional death over the age 65. So why don’t we try to prevent this? Unfortunately, due to reimbursement changes in health insurance home visits are not scheduled before discharge from the hospital, and home health is not always trained to address this issue. My additional accessibility training along with my Occupational Therapy background helps me to provide a unique perspective and give you solutions that help you to support your needs for years to come. I began Functionality For Life, LLC to help clients looking to age in place, remain independent regardless of abilities and remain a thriving member of their community.
